I/We assume the entire risks and hazards incidental to participation in MCBBA activities, including transportation to and from the activities. In addition, I/We do hereby waive, release, absolve, and agree to hold harmless the Mid-Cities Basketball Association, the organizers, sponsors, supervisors, officers, directors, participants, and persons transporting my child to and from any activities for any claim arising out of an accident, injury, or death sustained by my child.
